As we’ve been sharing over these last several months, to avoid these additional premium costs EVERY covered EMPLOYEE must complete BOTH the Personal Health Assessment (PHA) and a Biometric Screening BEFORE July 31, 2014.To complete the PHA, please log on to www.bcbst.com and go to Blue Access. To complete the Biometric Screening, a basic annual well visit (physical) must be done by the employee’s medical provider or on-site at one of the Biometric Screening events that have been scheduled at local terminals. Biometric Screenings may also be done at many of the clinics that perform DOT physicals for FirstFleet drivers.
